Please Note

This form should only be used by patients who have already been seen in a face to face initial appointment with one of our Weight Management Clinicians. The below form should be completed when you have 1 to 2 weeks’ worth of medication left, prior to a further dose being prescribed and posted to you via our pharmacy service. 

Please ensure this information is submitted in a timely manner to ensure your questionnaire can be reviewed by our clinicians, a new prescription issued (if appropriate) and time for this to be processed by the pharmacy for delivery.

If you have not yet been seen for an initial appointment, you can book this via our weight management page or by calling our team on 01227 202834 (Monday to Friday from 8am to 4pm).
