Our GP-led Lifestyle Medicine service provides education, counselling and practical support in a private outpatient setting. It is ideal for individuals seeking to improve metabolic health, reduce future health risks or address lifestyle factors contributing to existing medical and psychological conditions.
The aim of this service is to identify and address health risks that arise from long-term lifestyle patterns. Many of the most common and serious conditions seen in modern healthcare, including cardiovascular disease, type 2 diabetes, obesity, hypertension, dementia and certain cancers are strongly influenced by modifiable lifestyle factors. By proactively addressing these risks, it is possible to significantly improve both your current and future prospects.

What to expect at your appointments
Initial Lifestyle Medicine Consultation
Your first appointment is a comprehensive consultation with a lifestyle medicine GP (‘LM GP’) designed to understand you as a whole person - not just your symptoms.
Before you attend
You will complete a detailed lifestyle and health questionnaire. This allows your LM GP to review your medical background in advance and ensures your appointment time is focused, thorough and productive.
During your consultation
You will meet with a fully qualified LM GP who has additional accredited qualifications and training in Lifestyle Medicine (from the British Society of Lifestyle Medicine). This means your care is grounded in medical expertise and the latest evidence-based guidance.
Together you will explore:
- Your medical history and current health concerns, ‘what matters to you’
- Medications, investigations and risk factors
- Your personal goals and priorities
- Your core lifestyle pillars: nutrition, physical activity, sleep, stress and emotional wellbeing, social connection and exposure to harmful substances.
We take time to understand the root contributors to your symptoms and long-term health risks. Whether your goal is prevention of chronic disease, improving symptoms such as fatigue, or reducing the impact of an existing condition, your plan will be tailored specifically to you.
After your consultation - Personalised Health Plan
You will receive a clear, structured personalised health plan designed to support realistic, sustainable change - delivered with the reassurance of medical oversight and clinical expertise.
Follow-up Lifestyle Medicine Consultation
Follow-up appointments provide ongoing support, monitoring and careful refinement of your personalised health plan.
We will review your progress in detail - including how you are feeling, any measurable changes, and progress towards your goals. We will explore any barriers you have encountered and adjust your plan to ensure results are achieved safely, sustainably and in line with current evidence.
These LM GP follow-up consultations also allow us to review relevant measurements, medications and investigation, and to advise on any further testing if clinically appropriate.
Key Feature - Continuous Clinical Support
Lifestyle change takes motivation over time. All our packages allow patients to access secure messaging support between consultations, so you’re not left trying to navigate challenges alone.
You can send updates or questions through our secure online form and your LM GP will respond promptly. This provides reassurance, accountability and timely guidance as you implement your personalised health plan.
For safety, this service is intended for support and guidance related to your personalised health plan and does not replace your usual GP or any emergency medical services.
Extended Package
The Extended Package is designed to provide structured, ongoing support and monitoring as you implement and embed lifestyle changes over time. It allows space to explore more complex or detailed issues, monitor progress in symptoms and health markers, and refine your plan as needed.
This package includes three additional 20-minute GP consultations.
These follow-up appointments can be used flexibly to:
- Review progress and symptom improvement
- Adjust and optimise your personalised plan
- Introduce additional strategies in a stepwise, sustainable way
- Review measurements, medications or investigations where appropriate
The extended format supports accountability, clinical oversight and gradual, lasting change.

Scope of Service & Medical Disclaimer
This lifestyle medicine service provides education, counselling and support to help individuals optimise their health by making evidence-based lifestyle changes. The service focuses on optimising health to decrease chances of chronic disease, also on supporting individuals with established and medical diagnoses’ to address contributing lifestyle factors.
This service:
- Is not a substitute for medical diagnosis or treatment
- Does not provide acute medical care or emergency services
- Does not replace your registered GP or specialist care
If you have new, worsening, or concerning symptoms, you must seek assessment from your own GP or appropriate medical service.
Participation in this programme is intended for individuals who:
- Have already had appropriate medical investigation for ongoing symptoms, or
- Are seeking support for optimising their health, weight management, metabolic health, or risk reduction
You remain responsible for maintaining contact with your NHS GP and other healthcare providers regarding diagnosis, prescriptions, monitoring and ongoing medical management.
